All patients have some paperwork that must be completed before their first visit and then annually. For your convenience, we will send you registration paperwork electronically before your appointment via text and / or email. If you choose not to preregister, you may either come to the office 20 minutes before your appointment to register on a tablet or you can print out and complete the forms listed below.
